Services and Costs

Office visits, procedures and other healthcare services are listed in your statement for each visit at WellSpring. Specific therapeutic procedures, such as acupuncture, spinal manipulation and cupping, are not included within the tiered fee for the office visit itself.

Payment Policies

You are responsible for payment for all healthcare services provided at WellSpring. Payment is required at the time of your appointment other than those services for which we are billing your health insurance. Any deductible, co-payment or co-insurance portion is due at the time of service.

WellSpring Accepts These Forms of Payments

Besides using your medical insurance, care and services can be paid for at WellSpring in a variety of ways.

  • Credit or Debit Card (MasterCard and Visa)
  • Cash
  • Personal or Cashier’s Check

Insurance

Dr. Lori Beth Stargrove and Dr. Mitchell Bebel Stargrove are contracted as “In Network” providers with:

  • Regence Blue Cross Blue Shield
  • LifeWise of Oregon and Lifewise of Washington
  • Moda (formerly ODS)

Sara Snyder, LAc is contracted as an “In Network” provider with:

  • Regence Blue Cross Blue Shield
  • LifeWise of Oregon and Likewise of Washington
  • Moda (formerly ODS)
  • American Specialty Health (ASH)
  • Kaiser (CHP)

Many insurance plans also have an “Out of Network” benefit for naturopathic and acupuncture services.

WellSpring has many patients employed at Nike, Intel, Beaverton School District, Portland School District and other local employers.

The specifics of your policy and the riders selected by your employer will determine whether naturopathic and/or acupuncture medical care is covered, to what extent and under what terms. Our staff can help you verify your coverage.

In the event that your insurance does not cover our services, we recommend that you talk with your Human Resources department to advocate for real insurance equality, as mandated by law.
